Lucy Letby and the Thirlwall Inquiry
Doctors at the Countess of Chester Hospital warned managers about a nurse after a run of unexplained baby deaths. Managers were slow to call the police and told the doctors to apologise to her. The public inquiry into what went wrong reported in September 2026.

- Letby is moved off the neonatal unit
- Police investigation begins
- Convicted of seven murders
- Thirlwall Inquiry reports
Summary
- Lucy Letby was a nurse on the neonatal unit at the Countess of Chester Hospital, which cares for premature and sick newborns.
- In August 2023 a jury convicted her of murdering seven babies and trying to murder six more between June 2015 and June 2016. A retrial in July 2024 added a seventh attempted murder.
- She is serving 15 whole life orders, which means she will never be released.
- Consultants first raised concerns about her in 2015:
- She kept working on the unit until July 2016.
- Doctors first contacted the police in April 2017, and the police investigation began that May.
- The doctors who spoke up were investigated after she filed a grievance, and consultants were told to apologise to her.
- The babies' parents were not told about the suspicions until 2018.

Dig deeper
- Lady Justice Thirlwall's public inquiry reported on 15 September 2026. It found a "profound failure of management, governance and safeguarding" and called the way parents were treated "reprehensible".
- Its 17 recommendations include:
- a video monitor on every cot and incubator
- CCTV on the fridges where insulin is kept
- every hospital board tracking baby deaths by 31 March 2027
- The government replied the same day that it will change the law so senior NHS managers can be barred from working in the health service. Doctors and nurses can already be struck off by their regulators.
- The inquiry examined how the hospital responded. It did not rule on whether the convictions are safe.
- Letby's convictions stand. The Court of Appeal refused her permission to appeal in 2024.
- In February 2025 a panel of 14 international experts said they had found no medical evidence that she harmed the babies. No court has tested that claim.
- The Criminal Cases Review Commission, which investigates possible miscarriages of justice, is reviewing the case and has given no date for a decision.
- Prosecutors confirmed on 5 August 2026 that she will face no further charges.
Ethical issues
| Principle | How it applies here |
|---|---|
| Autonomy | Parents cannot decide anything for their child without the truth. Hospitals owe families that truth when something may have gone wrong, which is called the duty of candour, and these parents waited until the police made an arrest. |
| Non maleficence | Every doctor has a duty to raise concerns when patients are at risk, and to keep raising them if nothing happens. The consultants at Chester did, and the inquiry found that the hospital's leaders did not believe them. |
| Justice | Doctors and nurses answer to regulators who can end their careers, and until now NHS managers have not. The government's barring scheme is meant to close that gap. |
