What does a medicine reserve list mean?

At schools that keep one, a reserve list holds interviewees judged strong enough to be considered for medicine after the offers have gone to others. At Sheffield, which publishes the clearest policy, UCAS shows the application as unsuccessful, applicants are not told their position, and no reserve decision is made before A level results day.

A reserve list email arrives in the same weeks as offers and rejections, and it is neither. It means the interview went reasonably well, and it also makes the spring hard to plan. Our mentors stay with students through this stretch, because the choices made around a reserve list can cost an applicant a place they had already won.

What is a medicine reserve list?

A reserve list sits between an offer and a rejection, and schools handle that middle ground differently. Sheffield sets it out most plainly. Its policy, approved in 2022 and still the version on its website in October 2026, describes a ranked list of interviewees who did well enough to be considered but not well enough for an offer. The school ranks applicants by interview performance, the strongest get offers, and those "less strong, but still sufficient" are placed on the reserve list in rank order. Those below that line are rejected outright.

Other schools use other arrangements. Bristol's 2027 statement says it may hold some applications until later in the cycle, as late as May. Nottingham's policy for 2026 to 2027 describes offers going to the top candidates by interview score and does not describe a reserve list. So read each school's own wording before deciding what an email means.

Why does UCAS say unsuccessful if I am on a reserve list?

Because, at Sheffield at least, the application formally was unsuccessful. UCAS is the national admissions service, and Sheffield's policy is blunt about it: "Applicants on the reserve list have been unsuccessful in their application." Sheffield tells reserve candidates by email, and that email can arrive before UCAS shows the rejection.

When do reserve list offers come out?

At Sheffield, not before results day. The policy says no decisions can be made on the reserve list until A level results day, and that there is "no guarantee" any places will be free then. Places open up when offer holders miss their conditions or go elsewhere, and nobody knows how many until the grades are out.

A UCAS rule explains the timing. Under UCAS regulations, as Sheffield explains, a university "must not approach an applicant who is holding a firm or an insurance place at another institution". On results day, Sheffield advises reserve candidates who met its entry requirements to contact the medical school themselves.

Will I be told my position on the reserve list?

Not at Sheffield: "Applicants will not be notified of their position on the reserve list." Sheffield says the size of its list "varies each year", and it does not publish how many reserve candidates end up with offers, so any number on a forum is a guess.

How do firm and insurance choices work around a reserve list?

The firm choice is the offer an applicant accepts as their first choice, and the insurance is a back up in case they miss the grades on a conditional offer. UCAS allows one firm choice and one insurance choice, every other offer is declined, and the two cannot be swapped once results arrive. For 2027 entry, applicants outside UCAS Extra who have all their decisions by 31 March must reply by 5 May.

Sheffield's own advice is to let the reserve list play no part in that choice: an offer from another university, for medicine or any other subject, should be accepted or declined as if the list did not exist. We give students the same advice. A reserve list is one school's ranking, and treating it as an offer can lead an applicant to decline a place they could have kept.

Our Integrated programme includes standard support after offers, covering firm and insurance choices, Clearing and deferral, and Intensive extends it.

What happens if no offer comes?

Then the reserve list was a near miss. At Sheffield, reaching the list means the interview was strong enough to be considered for medicine, which points to different work for next time than a rejection before interview does. The next decision is whether to take another offer, go through UCAS Clearing, the system for filling places after results, or reapply, and what would change if they did.
